Welcome new clients!  I am truly looking forward to meeting you.  Please tell me a little more about yourself:

Before your first appointment, you may want to download these forms and fill them out:
Massage Client Information Form
Client Bill of Rights (you only need to read this)

If you are coming in because of a personal injury claim, you will want to print out some or all of these additional forms. Please fill them out and bring them with you to your first appointment.
*Financial Policy Agreement (required)
*Insurance/Attorney Lein (required)
Records Release Request (suggested)
Blank Referral Form (for your health practitioner, in case they want to use my form)

Appointment times are generally offered Tuesdays through Saturdays at the following times:
7:30am or 8:00am
9:30am
11:30am
1:00pm
3:00pm (Tuesdays only)
4:30pm (Tuesdays only)

You can also see where Susan's calendar is already busy - if I am gone, there is a good chance she'll be available during my absence. Gigi's busy spots are in green and Susan's are in purple.


It is possible to schedule online, once you are an established client. I will give you the code to access the "Appointments-ExistingCt" page, which has the links to my online scheduler (run by TimeDriver).

Once you are an established client, you will also receive a quarterly newsletter from me, which usually has an article on a health-related issue I've been studying recently, and any updates on special offers I might have.
